Best Mattress for Back Pain: Firm or Soft, and Which to Buy

The short answer

For most people with an aching lower back, a medium firm mattress is the best place to start: firm enough to keep the spine level, with enough give for the shoulders and hips. Side sleepers and lighter people usually need it a little softer; stomach sleepers and heavier people a little firmer. Very hard is not automatically better for your back.

Match firmness to how you sleep

The aim is a straight spine. If your hips sink lower than your shoulders the mattress is too soft; if there is a gap under your waist and pressure on your shoulder, it is too firm.

Mattress firmness by sleeping position
You sleep on yourStart withWhy
BackMedium firmSupports the lower back while letting the hips settle slightly
SideMedium to medium firmThe shoulder and hip need to sink in so the spine stays level
StomachFirmStops the hips dipping and arching the lower back
Different positions through the nightMedium firmThe safest middle ground for moving between positions

Body weight changes how firm a mattress feels

The same mattress feels firmer to a lighter person and softer to a heavier one. If you are on the lighter side, go one step softer than the table suggests. If you are heavier, go one step firmer and look for a zoned spring system, which puts more support under the hips.

Couples of very different builds should look at pocket springs or latex, which respond to each person separately.

Models to try, from firm to medium

These models state their firmness or are built to be firm, so they make a useful ladder to test in a showroom.

Which brand is best for back pain?

No brand is best for every back. What matters is the construction: a medium firm feel, a spring system that is firmer under the hips (zoned or centre-reinforced), and a comfort layer such as latex that supports without sinking. Every major brand makes at least one mattress built this way.

These are the support-focused models in each brand's range at MBF Mattress. Lie on two or three of them before deciding.

Test it properly before you buy

Lie on the mattress for at least ten minutes in the position you actually sleep in, not sitting on the edge. Ask someone to check that your spine looks straight from behind when you lie on your side.

This guide is general buying advice, not medical advice. If your back pain is severe, lasts more than a few weeks, or comes with numbness, see a doctor or physiotherapist before changing your mattress.

Reading only gets you so far. Come lie on them.

01What is the best mattress for back pain in Malaysia?

A medium firm mattress with zoned pocket springs or natural latex is the best starting point for most people with back pain. In Malaysia, support-focused models include Serta Support, King Koil's Chirotech models, Dunlopillo IcyLuxe, Dreamland's Chiro range and Getha's 7-zone latex mattresses. Lie on each for ten minutes before choosing.

02What mattress is best for back pain?

Medium firm suits most people with lower back pain: firm enough to keep the spine level, with enough give for the shoulders and hips. Look for zoned support that is firmer under the hips, and avoid mattresses that are very soft or very hard.

03Which bed brand is best for back pain?

No single brand is best for back pain. Serta, King Koil, Dunlopillo, Dreamland and Getha all make support-focused models; what matters is that the mattress is medium firm, supports your hips, and keeps your spine straight when you lie on it.

04What mattress should I buy if I have back pain?

Buy a medium firm mattress that keeps your spine straight in your usual sleeping position: slightly softer if you sleep on your side or are light, slightly firmer if you sleep on your stomach or are heavier. Test it for ten minutes first, and see a doctor if the pain is severe or lasts more than a few weeks.

05Is it better to have a softer or firmer mattress?

For most people, neither extreme: medium firm is the best starting point. It is firm enough to keep the spine level and soft enough to let the shoulders and hips sink in. Side sleepers and lighter people do better a little softer; stomach sleepers and heavier people a little firmer.

06Is a firm mattress better for back pain?

Not always. Medium firm suits most people with lower back pain. A mattress that is too hard leaves the lower back unsupported and puts pressure on the shoulders and hips.

07What firmness is best for side sleepers?

Medium to medium firm. The shoulder and hip need to sink in a little so the spine stays level.

08Is a coconut fibre mattress good for the back?

Coconut fibre makes a mattress firm and flat, which suits back and stomach sleepers who like a hard surface. Side sleepers usually find it too firm on the shoulder.

09How do I know my mattress is too soft?

Your hips sink lower than your shoulders, you wake with a stiff lower back that eases after you get up, or you roll towards the middle of the bed.
